Referral information
Referring your patients to Highgate Hospital is quick and easy.
Outpatient referrals
You can refer a patient directly to one of our consultants (for a directory of consultants see the Consultants A-Z). Alternatively, you can also refer direct to an outpatient service, where there is no named consultant our Outpatients team will select the most appropriate consultant to manage your patient's particular condition. You can refer using the following methods:
- Download a referral form (pdf 268KB) and fax to 020 8347 3892.
- Write your own letter of referral and fax to 020 8347 3892.
- Call Outpatients on 020 8347 3899 to make an appointment.
Diagnostic referrals
You can refer to a wide range of diagnostic imaging modalities including x-ray, ultrasound, extremity MRI and mammography. Experienced radiographers will undertake the examinations, with the report provided by a Consultant Radiologist and returned directly to you. To refer download a referral form (pdf 448KB) and fax it to 020 8347 3892 or give the completed form to your patient for them to make their own appointment with us by calling 020 8347 3866.
Physiotherapy referrals
You can refer to our team of experienced chartered physiotherapists for a wide range of conditions including back and neck pain, joint pain, limb and muscle conditions, sports injuries and post-surgery rehabilitation. To refer download a referral form (pdf 268KB) and fax it to 020 8347 3892 or give the completed form to your patient for them to make their own appointment with us by calling 020 8347 3899.

Patients can self-refer to Highgate Hospital for the following services:
